There are two registrations to complete on the national level. First, you need an NPI (National Provider Identification)6 number. Secondly, for financial purposes, including taxes and banking, you will need an EIN (Employer Identification Number)7 from the Internal Revenue Service (IRS) by completing this form.
Register your business with the Secretary of State in your home state. Make sure the business name you’ve chosen is available and consider if there is a domain name available with that name for your website too. Check with your city and county to determine if additional licenses are required.

Make sure your licensure is in order
To double check that you’ve met (or are in the process of meeting all of the requirements for operating a home healthcare agency in your home state, conduct a simple Google search for “home care license application” in your home state. If you haven’t yet, once you have completed your state’s home healthcare application for licensing, keep in in mind that if you plan to receive payment from Medicare or Medicaid plans, you must complete the licensed and certified (L&C) home health care application.
Note that each state is different. Some states impose more or stricter regulations than others. Application fees can range from $500 to $5,000. Some states require waiting periods or may not be issuing new licenses. Others may offer a provisional license that requires you to service clients before obtaining full licensure. Remember, you cannot bill many of the payors until you are licensed and certified.
